SERVICES OFFERED

PHS offers vaccines for babies and kids (up to 18 years old) weekly at our Crete office, Mondays 3-6pm by appointment (NO WAITING!). Call 888-310-0565.

COST OF SERVICES

$13.50 for a single vaccine (administration fee)

$10.50 for each additional vaccine (administration fee)

The Vaccine for Children program does accept Medicaid

Adult Vaccines are available. Adult price varies based on specific vaccine
