Medicaid Assistance
P.H.O.N.E. - Public Health Outreach Nursing Education
This program is contracted by the Nebraska HHSS and administered by Public Health Solutions to provide community outreach and education to clients receiving or potentially eligible to receive Medicaid or Kids Connection Health Care Coverage.
The Public Health Nurse will assist eligible individuals and families by:
- Helping find medical, dental, and vision homes
- Identifying barriers, such as transportation and language that prevent the family from obtaining healthcare services
- Offering information on community resources and assisting with referrals as needed
- Educate about medical home care concepts, preventive health measures, and access to health care
